We are pleased to welcome you to Beeville Family Practice L.L.P. The physicians, mid level providers, nurses, and all of our staff are committed to providing our patients the highest quality health care in a safe and comfortable environment.

You are encouraged to carefully read this brochure, as it will answer many questions you may hove regarding the care and services available at Beeville Family Practice L.L.P. We appreciate the opportunity to assist you in your health care needs . We are committed to do our best to meet your medical needs and concerns.

Beeville Family Practice L.L.P. is dedicated to provide the most modern medical care available with a compassionate and caring approach that focuses on the individual. We make every effort to make sure that you understand your medical condition and the treatment options. We encourage you to ask questions if something is not clear to you.

Physicians rotate evening, weekend, and holiday calls to assure that a physician is available to patients under our care at all times.

Our providers try their best to stay on schedule. If they are seriously delayed, we will attempt to notify you as soon as possible.

Should you have questions regarding your condition, medication, or treatment, please call (361)358-9912 during normal business hours so we may have access to your medical records.

If you need to have your prescription refilled, it is helpful if you know the name of your pharmacy. If you have your prescription bottle, it is best to have it handy when speaking with our office staff. If you are on a maintenance medication, (diabetes, hypertension, heart, lung etc.) you may have the pharmacist call us directly for possible approval.

BILLING AND INSURANCE
If your insurance plan requires a flat co-payment amount for each visit, you will be expected to pay it upon arrival.

If you are covered by a private insurance carrier we ask that you pay for the first visit. Private insurances vary and may cover anywhere from zero to 100% of your medical charges. We offer several payment options including Master card, Visa, Discover and personal checks.

Family practice is the medical specialty which provides continuing and comprehensive health care for the individual and family. The scope of family practice encompasses all ages, both sexes, and each organ system.

EMERGENCY CARE
Should you have an urgent problem arise while the office is open, please contact our office at (361) 358-9912 ext. 0 and the doctor will be in touch either directly or through office personnel giving you instructions for care.

If the situation arises while the office is closed, the answering service will answer and promptly forward your message to the doctor on call. You will receive a response as quickly as possible.

In the event of a true emergency, it is best to go directly to the nearest Emergency Room where the physician on duty will initiate treatment immediately.

NEW PATIENT VISITS
Upon your first appointment and each visit thereafter with Beeville Family Practice L.L.P., you are requested to bring your insurance card(s) and all your current medications this Includes all prescribed medications, all over the counter medications, vitamins, supplements and herbs etc.

We will mail you a patient registration form and health questionnaire to fill out before your visit. We ask that you come 15 minutes early so we may copy your insurance Information and card and verify the information you have provided.

CONFIDENTIALITY
Your medical record is strictly private. We will not give out information regarding your condition to your employer, friends, or relatives without your written authorization. The only exception to this is when required by law.
